We wish to know the name of your husband, the time and place of the marriage, the names and ages of the children (if any), the time during which you have resided in Iowa and the place of such residence, when the husband left and the alleged cause of his leaving and where he was when last heard from and the time you last heard from him, whether since leaving, he has contributed to the support of the family and the kind and amount of such support (if any) also what property he owns in his own name.
